Documents that will be required as part of the enrollment process:
1. Copy of Birth Certificate
2. Copy of Baptismal Certificate (if baptized Catholic)
3. Complete immunization records and/or vaccination exemption letter from physician
4. Report of Physical Exam from Doctor (K, 6th, transfers)
5. Report of Dental Exam from Dentist (K, 3rd, 7th, transfers)
6. Create STS account for tuition management and payment plan options at https://app.simpletuitionsolutions.org/register?sc=20626
7. Submit a financial aid application through STS (if interested in tuition assistance)
